External review is available to anyone who has received a final denial of a request for the provision of or payment for a health care service or course of treatment from an insurance company. You may have the right to have your denial reviewed by health care professionals who have no associate with your insurance company if the decision involved making a judgment as to the medical necessity, appropriateness, health care setting, level of care or effectiveness of the health care service or treatment you requested. You may submit a request by completing an external review application and sending it to the Life and Health Actuarial Division.
